A breakdown call should start the diagnosis, not a search for basic context. The useful information is rarely a perfect technical explanation. It is a clear account of what changed, what the machine was doing and what has already been made safe. Preparing that handover can shorten the path from arrival to informed testing without encouraging anyone to bypass the site’s safety process.
01 / 03
Make the plant safe and preserve the evidence
Follow the site’s emergency, isolation and escalation procedures before gathering information. Do not repeatedly reset protection, clear alarms or dismantle equipment simply to see whether the fault returns. Those actions can remove useful evidence and may expose people or plant to further risk.
Where it is safe and authorised, retain alarm history, note the machine state and photograph visible damage from outside controlled areas. The technician needs to know what has changed since the event as much as what happened during it.
02 / 03
Tell the operating story in order
Describe the last known normal condition, the first unusual sign and the final plant response. Include product, speed, load, changeover activity, recent work and whether the same symptom has appeared before. A short sequence is more diagnostic than a broad statement such as the machine just stopped.
- 01Equipment name, line and exact location
- 02Alarm or protection indication before it was cleared
- 03Operating condition immediately before the event
- 04Recent maintenance, adjustments or product changes
- 05Current isolation and access status
03 / 03
Nominate the person who can make decisions
Arrange site access, inductions and the person authorised to explain the process and approve the next step. A technician waiting for keys, permits or production direction is not diagnosing the plant.
Agree how production, maintenance and the attending trades will communicate status. One factual handover prevents the fault story from changing each time it passes between people.
A good callout brief does not diagnose the fault. It gives the diagnostic work a safe, informed starting point.
